BABY DELIVERY MUST HAVE: DOCUMENTS YOU NEED TO PREPARE.

For this blog, I will share with you the documents I prepared for my hospital admission (baby's delivery). Truth be told, I was extremely excited preparing my hospital documents - I guess that's the happiness of an OC person.
YOU (MAY) NEED THE FOLLOWING:
✅ Clear book (Size of your preference. I bought the legal size).
✅ Sticker paper (Size of your preference. I bought A6 Sticker Paper in Matte via Shopee).
✅ Bond paper.
✅ Laptop or computer (For use in making labels and other documents you want to lay out or if laptop/computer is not available, you can pay for lay-out services near you).
You may use your smartphone or notepad/iPad, however, expect a more arduous process.
✅ Printer (For printing documents and labels, if not available just pay for printing services near you).
✅ Photocopier (For photocopy of your original documents, if not available just pay for photocopy services near you).
Before I resigned from work, I laid out and printed labels, name tags, and lists for my hospital bag, and my expectant mother's information sheet. I also photocopied all the original copies of my ultrasound and medical records at work. I was grateful for the opportunity. P.S. I used my own bond paper and sticker paper.
For your labels, you can lay them out using Canva, if you like your labels with designs.
Below are the documents I prepared for use in hospital admission/release:
☑️ ULTRASOUND & MEDICAL RECORDS PHOTOCOPY
Make sure you put this on the first page of your clear book for easy access. Arrange them from the latest to the oldest.
☑️ EXPECTANT MOTHER'S INFORMATION SHEET
This document contains your personal details and important information. Based on my experience, information about you and your pregnancy will be asked whenever you go to the hospital. Consider this document as your husband/partner/watcher cheat sheet and for yourself (as well) during this time when your hormones messed up with your memory.
Note: Never forget your last menstrual period (LMP) and Expected Delivery Date (EDD), these two are often asked.
During admission, possibly you are in active labor (in terrible pain, yes), nurses and doctors in the emergency room will ask you so many questions. If you can't handle interviews and active labor at once, you can kindly refer them to your expectant mother's information sheet or to your husband/partner/watcher if he/she is allowed inside the ER.

☑️ ULTRASOUND (ORIGINAL COPY)
Make sure to arrange your ultrasounds from the latest to the oldest. During my admission, the resident doctor asked for my first ultrasound, the transvaginal scan.
☑️ MEDICAL TEST RESULTS
The medical test depends on your condition and your OB's request. But for reference's sake, I've listed below the medical test results I put in my clear book. I arranged them according to the list below and from the latest to the oldest.

🔸Glycosylated Hemoglobin (Hb A1C)
🔸Oral Glucose Tolerance Test

🔸Serology/Immunology (HBsAg)

☑️ PRESCRIPTIONS (OB-GYNE)
When I went to Brokenshire Hospital emergency at that time, the attending physician (not my OB) inquired about the supplements and prescriptions I was taking. I was loading, since my hormones were too fault. I made a pledge to myself after that occurrence to keep all of my prescriptions together (in my clear book).

🔸PhilHealth Member Data Record
🔸COVID-19 RT-PCR related forms/documents.
☑️ IDs PHOTOCOPY
🔸PhilHealth ID Card
🔸COVID-19 Vaccination Card
🔸Watcher's COVID-19 Vaccination Card
🔸BCP 1 ID Card (for admission in Brokenshire Hospital, proof of availed hospital package)
🔸2 Valid IDs (I used my UMID & PRC ID)
☑️ CERTIFICATE OF LIVE BIRTH CHEAT SHEET
If you deliver your baby at the hospital, the hospital will take care of your baby's certificate of live birth (submit to LCR) but you need to fill out some information.
Why do you need a cheat sheet?
Well, you just gave birth, in pain, disoriented, and taking care of your newborn. The last thing you will do is process your baby's birth certificate yourself.
Give the cheat sheet to your husband/partner/watcher so he/she can process it on your behalf. It is important that correct information is filled out and you cannot expect others, yes even your husband, to do it perfectly.

☝Processing of birth certificate in the hospital is needed so your baby can avail of the PhilHealth discount.
☝If not married but the baby will be acknowledged by the father, you must bring your baby's birth certificate to an attorney for the notary. The hospital will give you at least 3 days to do this (Brokenshire Hospital for this matter) so you can still discount your baby's bill via PhilHealth.
☝You will be the one to process your baby's birth certificate certified true copy. You can do this after 1 month from when the hospital submitted the original copy of the baby's birth certificate to LCR.
☝Birth certificate certified true copy will be used in the processing and claiming of your SSS maternity benefit.
☑️ OTHER DOCUMENTS
🔸Hospital package details
🔸Marriage Contract
🔸Receipts
🔸Documents received from the hospital
🔸Admission request from your OB (my OB gave my admission request during our last check-up at 38 weeks)
🔸Results of COVID-19 RT-PCR test (your own and your watcher if required)
That's it! Hope this will help you in preparing your hospital documents. You can add other documents I did not mention above. Remember, it's best to prepare your hospital documents early. Giving birth is like a surprise party (except when you are for scheduled CS). You have to be prepared every time!
☝Make sure to inform your husband/partner/watcher about your hospital documents. Let him/her scan your clear book and familiarize what's inside.
☝For backup, I advise you to create soft copies (use your phone and take a picture) of all your hospital documents, particularly your ultrasound and medical records. Create folders to put documents in one place. You can access them easily whenever the need arises.
Remembering my own delivery, it was indeed a busy day! I was thankful, to my watcher, and the hospital personnel (I believe) that I had prepared the documents needed for hospital admission and release. Everything went smoothly for all of us.
When you are prepared, you will have more time for what is most important.
